How to Register & Complete Your Health History Forms

  • Use the link in your welcome email to access our Living Matrix self-registration page. 
  • Please fill in each section accurately, and choose an email that you check regularly. We send all health history forms, follow-up forms, lab reports, recommendations and handouts through Living Matrix, and you will be notified via the email you select.
  • To verify your identity, Living Matrix will send you an activation code, either by text or to your email. If you prefer to receive this code via email, please uncheck the box “Send activation code by text Carrier charges may apply per your messaging plan”

How to Login to Your Living Matrix Account

  • NOTE: Living Matrix is ONLY accessible via a desktop or laptop computer. It is not compatible with tablet or mobile devices. For the best experience, please use Safari (Mac), Mozilla Firefox, or Google Chrome as your web browser. Internet Explorer and Edge users may experience technical issues.
  • Please visit amybeard.livingmatrix.com or copy and paste that link into your browser. We recommend bookmarking this URL for easy access in the future.
  • In the “User Login” window, type in the email address that was used to set up your profile in the system.
  • Add your password and click the “Login” button.

I Forgot My Password

  • Go to your practice’s login page amybeard.livingmatrix.com
  • Click on “Trouble Logging in?”
  • Enter your email address and click “Email password reset instructions.” An email will be sent to you from no-reply support@livingmatrix.com. Open the email and follow the instructions to create a new password.

I Can No Longer Access the Email Associated with My Account

  • Please contact our admin team to have them update your email in Living Matrix.
  • Once they have updated your email, you will receive an email to confirm this update. You must confirm the update in order to receive future emails to that address.

Accessing Your Care Plan & Lab Results

Via Email

  • Once your Care Plan is complete, you should receive an email from Living Matrix with the subject, “Patient Summary Report Shared.” It will come from this email address: no-reply+support@livingmatrix.com. That email will give you a link to login and view your report.
  • Please check your spam box if you feel you didn’t receive this email. You can also try searching for “Patient Summary Report” in your email to see if that locates the email.
  • Login using your username (which is your email) and password.
  • Once logged in, access the Care Plan by clicking on the “Reports” tab on the left.
  • Click the “View PDF” button next to your Care Plan document. This will open your care plan in a new tab. From there, you can download, save and/or print your plan for your reference. Your plan will always be stored in your Living Matrix profile as a shared report.

Via Your Account

  1. Login to your Living Matrix Account. Go to http://amybeard.livingmatrix.com/ to login.
  2. Login using your username (which is your email) and password.
  3. Once logged in, access the Care Plan by clicking on the “Reports” tab on the left.
  4. Click the “View PDF” button next to your Care Plan document. This will open your care plan in a new tab. From there, you can download, save and/or print your plan for your reference. Your plan will always be stored in your Living Matrix profile as a shared report.

**Please note: The first date associated with the document may look confusing, as it is the date your profile was established. The date SHARED is the date that the Care Plan document was sent to you.
