If this is your first time joining our mailing list, you should receive an initial email in 5-10 minutes. You may need to check your spam box and then add firstname.lastname@example.org as a contact! Feel free to reach out to our office at email@example.com if you have questions or concerns.
Getting Set Up as a New Client
Step 1: View and sign our New Client Consent Forms electronically.
Step 2: Complete the Living Matrix health history forms.
Click here to register for a Living Matrix account. Follow the prompts to set up your account. Once you are set up and logged in, your profile will be pre-populated with 3 forms. These forms must be completed on a desktop or laptop computer, as this program is not compatible with tablets or mobile devices. Please use this page as a resource should you have technical difficulties.
Depending on the complexity of your health history, these forms may take up to 2 hours to complete. You may save the information and return later if needed. Be sure to hit “submit” after completing each form. (*Please save your password! You will use this account to access follow-up forms and Care Plans).
**YOUR PAPERWORK MUST BE COMPLETED 1 WEEK PRIOR TO YOUR INITIAL HEALTH & MEDICAL INVESTIGATION.
Step 3: Schedule your initial consultation.
If you haven’t scheduled already, please book your Initial Health & Medical Investigation on our client scheduler. All new clients MUST start with an initial health & medical investigation. If you have any questions, feel free to reach out to our admin team at (501) 581-1212 or firstname.lastname@example.org.
This 60-minute consultation will be with one of our Functional Medicine Consultants. Upon scheduling, you will be prompted to pay online with a credit card. If you would like to pay with a Health Savings Account card (HSA card), please call our office to schedule as our online scheduler may not accept your HSA card.
Our practice does not take insurance for our consultations, but you may use your insurance for any standard labs that may be needed (AR and MS residents only). Please check with your insurance provider to understand your coverage.
Step 4: Download Zoom on your computer.
All consultations are done via our HIPPA compliant virtual meeting room, Zoom. Don’t worry! It’s very easy to navigate. And you can ‘attend’ your appointment from anywhere.
Visit https://www.zoom.us/ and sign up for free. Follow the prompts to download the free Zoom platform on your computer. You will receive an email prior to your appointment with a link to your personal meeting room. Please click that link 10-15 minutes prior to your appointment to join the meeting. Please use this page to try a test Zoom call and/or review helpful tutorial videos.
